What this notice covers
This notice explains how Thrive Wellness MD handles the information you share when you take the Thrive Score, the free wellness assessment on this website. Everything else on our website is covered by our general Privacy Policy.
What we collect and keep
When you complete the Thrive Score, we keep your full name, your email address, and a record of which consent boxes you checked and when. That is the complete list. Your answers are used only in the moment, to calculate your score and create your report, and they are not saved after your report is created; the same is true of the scores themselves, which live in the report we email you rather than in our systems. We do not collect your street address, phone number, precise location, or payment information, we do not store your IP address, and there are no free-text boxes. Nothing reaches us at all until you submit your answers at the end of the assessment.
What we use it for
Your answers are used to calculate your Thrive Score and to create the report shown to you and emailed to the address you provide. That email is your copy of your results, since we do not keep them. If you checked the optional box to receive educational health content, we will use your name and email to send you occasional educational emails. You can always opt out if you change your mind in the future: every email includes an unsubscribe link that works immediately, and writing to us at Drmohammadi@thrivewellnessmd.care does the same. The box is not required, and leaving it unchecked changes nothing about your score or report.
What we never do
We do not sell your information, and we do not share it with anyone for advertising. There are no advertising trackers or third-party analytics on the question or results screens; our website's ordinary visitor analytics runs only on the page that introduces the Thrive Score. Your answers are never added to any medical record, including at our own practice, and no one will contact you about your individual answers.
Who sees your answers
No clinician or staff member reviews your individual answers, and the Thrive Score is not a monitored service. Your score and report are generated automatically, and because your answers are not saved, there is nothing for anyone to review afterward. As with any secure database, the small number of administrators who maintain our systems could technically access what we do keep, your name, email, and consent record; they work under confidentiality obligations.
Where your information lives, and for how long
What we keep is stored in systems our practice controls with our cloud hosting provider, Amazon Web Services, in the United States, encrypted while stored and while traveling. Your record deletes automatically 24 months after you complete the assessment, and deleted records can persist in routine system backups for up to 35 days before those age out as well.
Your choices
You can ask us at any time for a copy of your information, or ask us to delete it sooner than the automatic schedule, by writing to Drmohammadi@thrivewellnessmd.care from the email address you used. We honor these requests wherever you live. You can unsubscribe from educational emails with the link inside any of them.
For adults
The Thrive Score is intended for adults 18 and older, and we do not knowingly collect information from anyone younger.
Not medical care
The Thrive Score is educational wellness information. It is not medical advice, diagnosis, or treatment, taking it does not make you a patient of Thrive Wellness MD, and it cannot rule out medical problems. If you have symptoms or concerns about your health, please talk with a clinician who can evaluate you personally. If you are struggling, call or text 988 to reach the Suicide and Crisis Lifeline, and in an emergency call 911.
Changes and questions
